Most, if not all the selected PDF form can be filled by typing in the information and then printing it out.  Any informational pages within the selected PDF form, do not have to be printed out for your Notary appointment.

DO I HAVE TO USE A NOTARY TO COMPLETE THESE FORMS?
affidavit formsYes,  these forms must be signed by a notary public.  Any Living Will and/or Durable Mental Health Care Power of Attorney may need a witness, whom is at least 18 years old, cannot be a family member, cannot be in your will to receive part of estate, cannot be appointed as your representative, and cannot be a health care giver. A witness can be a neighbor, a friend and/or any adult that is not in your will and not caring for you and/or representing you.

General Affidavit Form

Living Will Power Of Attorney form – “Making decisions and/or or advance choices today, in writing, about what medical care you want if ever in a terminal condition, persistent vegetative state and/or irreversible coma”

Durable Health Care Power Of Attorney form – “Selecting a personal to make any future health care decision for you in the event you become to ill and/or cannot make those decision for yourself”

Advance Directives For Health Care Planning form – “Having a heath care representative appointed by a person in writing or if none personal has be appointed, an appointed representative according to the law. The Advance Directives For Health Care Planning form, but not limited to, is mailed and registered into the Arizona Advance Directive Registry”

DMV Power of Attorney form – “To appoint a Attorney-In-Fact to sign all documents required to secure title and/or authority to transfer title”

DMV Special Military Exemption form – “Special Military Exemption is a one time per deployment that applies to no more than two vehicles and each vehicle would require a separate Special Military Exemption form. Military orders must be presented at the time of application”

DMV National Driver Register Check form – “For Employers, Commercial Air Carrier, Railroad Company to authorize a one-time National Driver Register (NDR) check”
